I certainly hope you are enjoying your day and taking a moment to reflect on the amazing work of Dr. Martin Luther King, Jr. Our Wind Ensemble students performing a very beautiful piece of music at the assembly held last Thursday to honor the man, and the impact he had on our country, and really individuals around the world.

FORMS FOR TRAVEL - EMERGENCY/PERMISSION & MEDICINE

As we get ready for a bit of a travel season, it's time to ensure everyone has some of the emergency, medicine, and permission forms that are needed. Use the links below to access the forms you might need. These are also found on our meadbands.org/forms page.

The "Athletic / Activity Emergency Form" listed below is needed by everyone one time annually. That's right - the good news is we only do this one time per year and this form will serve also serves as our annual permission slip for all activities band related!
